Asthma & COPD

1. Asthma
   1. Send Asthma Control Test (via accuRx Florey)
   2. Need to document a peak flow
   3. Need to ask about inhaler technique
   4. Need to ask how many flare ups in the past year
   5. Need to ask if they have hayfever
   6.  Then book r/v appt

1. COPD:
   1. Need to document the MRC score, and if greater than or equal to 3 will need pulmonary rehab ref
   2. Send the COPD (CAT) questionnaire via Florey
   3. Need to ask number of exacerbations in the past 12m
   4.  Then book r/v app
