Book appointments with Dr Meade for annual review in their birth month

Learning Disability (HI-01)
The health action plan must be recorded on or after the date of the annual
health check
PCA can be manually applied for LD patients that refuse the offer of a health check (clinicians are advised to assess the patients mental capacity if the patient refuses)
In addition to Network DES payment, practices will receive an item of service payment (£140) for each annual health check done
